IT IS A GENUINE USAGE, THE COVER WITH INSTRUCTIONS ON REVERSE: "IF DR. KANE HAS LEFT VERA CRUZ SEND TO PHILADELPHIA, PENNA."

The pencil marking is probably Pd, indicating the original mailing from Philadelphia on MAR 8 was paid in cash. The US stamps were probably applied in Mexico just before the cover was forwarded back to Philadelphia. They were not applied at the US border since they were overwritten by the forwarding address. The MAR 8 mailing date does not apply to the stamps.

Elisha Kent Kane (February 28, 1820 – February 16, 1857) was an American explorer, and a medical officer in the United States Navy during the first half of the 19th century. He was a member of two Arctic expeditions to rescue the explorer Sir John Franklin. He was present at the discovery of Franklin's first winter camp, but he did not find out what had happened to the fatal expedition.
His time in Mexico dates from Nov 6, 1847. He sailed from New Orleans to Vera Cruz on Nov 23 on the USS Fashion, which was caught in a storm and finally arrived in Mexico on Jan 1, 1848. By late Feb he was in Mexico City. He returned home to Philadelphia in the summer of 1848. In 1849, still in the US Navy, he sailed to Europe. Therefore the cover was mailed back to Philadelphia in the Spring of 1848.
